Each schedule received by the Registry of Births, Deaths and Marriages was registered with an annul single number according to date of receipt (Schedules received in 1932 were registered by annual single number with 'M' (Melbourne) prefix.

Each schedule contains the following information:

    - When and where died.
    - Usual place of residence.
    - Name and surname.
    - Occupation.
    - Sex and age .

    - Duration of last illness.
    - Legally qualified medical practitioner by whom certified.
    - When he last saw deceased.

    - Signature of Registrar, date and where registered.
    - When and where buried.
    - Undertaken by whom certified.
    - Name and religion of Minister, or names of witnesses of burial.
    - Where born, and how long in Australian states.
    - If deceased was married, where, at what age and to whom.
