  • How to use the Records
    These records were sentenced as temporary records under Public Record Office Standard 09/07, class 1.4.1, and destroyed in 2014.

  • Function / Content
    This series consisted of bound volumes containing information forms for the registration of deaths in the state of Victoria. The information was transcribed into VPRS 8340 Death Register.

    On 1 October, 1960 the "Registration of Births, Deaths and Marriages Act" 1959 came into operation, allowing registration of births and deaths to be effected by post instead of personally by attendance before a Registrar of Births and Deaths. The Act combined the registration of both metropolitan and country births and deaths. Each information form for the registration of deaths received at the Registry of Births, Deaths and Marriages was registered with an annual single number by date of receipt.

    Information forms provide the following details:

    -date of death
    -place of death
    -usual residence
    -name
    -marital status
    -usual profession or occupation
    -particulars if a pensioner
    -sex
    -age
    -name and address of legally qualified medical attendant who certified the death
    -name and address of father and mother of the deceased
    -birthplace of the deceased
    -how many years in Victoria, other Australian states
    -name and address of undertaker
    -date and place of burial or cremation
    -if deceased was married, where, at what age, to whom
    -names and age of issue in order of birth
    -name and address of person supplying the information, and relationship to the deceased
    -if the deceased was an ex-member of the armed forces, particulars of the unit to which he/she belonged
